How to add a contact form in the page builder

Learn how to add and configure the contact form block in the page builder — including form settings, field selection, and what happens after submission.

The contact form block is a quick way to collect visitor information and generate leads directly from any page. When someone submits the form, they're automatically added to your Contacts (CRM) with a tag so you can follow up.


Step 1: Add the contact form block

  1. Open your page in the page builder.

  2. Click "Add content".

  3. Select "Contact form" from the right-hand menu.

Once the block is on your page, the settings are split into three tabs: Form, Fields, and After submit.


Step 2: Form tab — title, labels, and settings

The Form tab controls what visitors see on the form itself.

Form title and description

  • Form title — the main heading displayed above your form. Required.

  • Text below title — a short description or instruction for visitors. Required.

Submit button label

Change the default "Submit" button text to something more fitting for your page — for example, "Get in touch", "Send message", or "Request a demo".

CAPTCHA protection

CAPTCHA is enabled by default to protect the form from spam submissions. Toggle it off if you don't need it.

Privacy / integrity policy link

Because you're collecting personal data, a privacy policy link is required. Your default policy link is pre-filled automatically. You can replace it with a different URL if needed.

To create or edit your privacy policy, go to Settings → Accounts & Policies → Integrity policy.


Step 3: Fields tab — choose what to collect

The Fields tab lets you control which information the form asks for.

Always collected (required, cannot be removed)

  • Name

  • Email

Optional fields (toggle on/off)

  • Title

  • Phone number

  • LinkedIn profile

  • Company name

  • Corporate identity number

  • Website

Click the toggle next to each field to enable it (green = visible on the form, red = hidden).


Step 4: After submit tab — what happens next

The After submit tab controls what the visitor sees after submitting, which tag is applied, and whether you receive a notification.

Confirmation message or redirect

Choose one of two options:

  1. Show a confirmation message — replace the form with a thank-you message after submission. Enter your message in the "Message shown after submit" field.

  2. Redirect to another page — send the visitor to a different URL (e.g. a thank-you page or the next step in a funnel). Paste the URL into the "Link to redirect to after submit" field.

Lead tag

Every submission automatically gets the system tag ContactForm. You can also assign an additional tag to help you segment submissions — for example, by campaign or landing page.

  1. Click inside the "Lead tag to assign after submit" field.

  2. Choose an existing tag from the dropdown, or start typing to create a new one.

You can assign one additional tag per form. The ContactForm system tag is always applied regardless.

Email notifications

Toggle on "I'd like to be notified when someone submits the form" to receive an email for every new submission. This is on by default.


Step 5: Save the form

Once you've configured all three tabs, click "Save" in the top right corner. Your contact form is now live on the page. 🎉


Where do submissions go?

Every submission creates or updates a contact in your CRM. To find them:

  1. Go to Contacts in the sidebar.

  2. Filter by the ContactForm tag, your custom lead tag, or search by name or email.

💡 Submissions from the contact form block do not appear under Toolbox → Forms → Submissions — that's only for regular forms. Contact form submissions always go to Contacts.


Troubleshooting

  • Not receiving notification emails? Check that the notification toggle is on in the After submit tab.

  • Can't find submissions? Check Contacts, not Toolbox → Forms — contact forms store data in the CRM.
